Outcome monitor
Also called: decision monitoring, post-decision review
Definition
An outcome monitor is a commitment, made when a decision is adopted, to check later whether it produced the intended result — with a metric, a target, an owner, and a report date. It turns a vote into an accountable experiment. Boards that skip it often re-debate decisions without knowing whether they worked.

A decision can carry outcome monitors with a title, metric, target, status (planned, active, report due, reported, complete, or waived), due date, and last report, plus a revisit date. Decision outcomes at /app/decisions/outcomes lists them; it does not judge whether a target was met, because no stored field records that.
